21 Conclusion SIST-MC uses best available population data and takes into account their geographic and environmental features SIST-MC provides capacity for analyzing epidemiological data, revealing trends and interrelationships that would be more difficult to discover in tabular format SIST-MC allows policy makers to easily visualize chronic diseases problems in relation to existing health and social services and their natural environment

22 Conclusion SIST-MC can be used to properly plan, manage and monitor chronic diseases public health program, providing relevant information to decision-makers at all levels of the public health system SIST-MC can be accessed online at low cost SIST-MC has been approved by the commission daccès à linformation of Quebec and by the ERB of the CHUS
