La question clinique Chez les sujets avec MTE et cancer, est-ce que l’édoxaban (Lixiana au Canada) est aussi efficace et sécuritaire qu’un traitement de daltéparine (Fragmin) ?

Méthode Devis : essai clinique randomisé de non-infériorité (ouvert). Population : 1050 adultes avec TPP proximale ou EP (segmentaire ou plus grosse) symptomatique ou non associée à un cancer actif ou diagnostiqué dans les 2 dernières années.

Méthode Intervention : HFPM pour au moins 5 jours puis Édoxaban 60 mg PO DIE. Comparateur : Daltéparine 200 UI/kg s/c DIE x 1 mois puis 150 UI/kg s/c DIE. Objectifs : critère de jugement primaire (issue primaire) : combinaison de MTE récidivante + saignements majeurs à 12 mois.

Méthode Suivi : 12 mois. Suivi des patients = 99,2 % Sites : 114 centres dans 13 pays (Canada, USA, France, UK, etc.) Financement : Daiichi Sankyo.

Conclusion Chez les patients avec maladie thromboembolique et cancer, l’édoxaban oral est non-inférieur à la daltéparine sous-cutanée, le traitement standard, sur la combinaison de maladie thromboembolique veineuse récidivante ou de saignement majeur. L’incidence de saignement majeur est plus grand avec l’édoxaban.
