R-CHOP A vs CHOP B R j1 CHOP j3 + 3 R “entretien” 95 vs 47 patients Complete response (CR+CRu )was achieved in 55/95(58%) and 23/47(50%) patients in arms A and B respectively (p=.371) Death due to infection occurred in 7/95 (7%) group A (including sepsis in 6) and only 1/47 (2%) group B patients. Documented infection in the setting of neutropenia occurred in 10 and 0 patients in groups A and B respectively (p=0.031) En fait essentiellement chez des patients ayant moins de 50 CD4 / µl à l’inclusion

Fréquence des différents cancers dans Oncovih 2006 et dans Mortalité 2005
Quand la barre noire est plus haute que la barre orange, cela signifie que l’on ne fait pas bien en terme de survie et qu’il doit y avoir des progrès dans la prise en charge de patients, ou bien pas bonne représentation (ex : hépatocarcinome). C’est le cas pour les lymphomes, les hépatocarcinomes, et le cancer du poumon en raison d’une prise en charge plus tardive. Etonnant MH et anus ?

Conclusion Le traitement des lymphomes associés au VIH rejoint celui des VIH- Avec un pronostic qui s’en rapproche ??? Oui pour certains Non pour les autres…. Les facteurs pronostics sont principalement CD4 et IPI

Et CD4 > 100 Pour les IPI 2-3 il faut trouver mieux Intensifier ? R - ACVBP? ABMT? Pour les CD4 < 50 ou 100 Place du Rituximab ? Intensité??? Le Burkitt “grave”???
