Adding brief lifestyle counseling, delivered in eight 10- to 15-minute sessions, to sibutramine resulted in a weight reduction of 7.5 ± 8.0 kg at 1 year. When the 30-week group-based lifestyle modification program was added to sibutramine, however, participants achieved a 1-year weight loss (12.1 ± 9.8 kg) that was nearly double that of either therapy alone. These findings demonstrate that the effects of lifestyle modification and pharmacotherapy are equivalent when used separately, but additive when used in combination.As with behavioral treatment, pharmacological weight loss therapies should be continued long term to facilitate the maintenance of weight loss. In studies of sibutramine and orlistat, significant weight regain has been found upon discontinuation of the drug. More results from prospective studies are needed to address the question of whether the presence of the metabolic syndrome has an impact on the global CVD risk that is assessed solely on the basis of traditional risk factors. However, the significant improvement in hypertension seen at 2 years was no longer present at 10 years.
